Insurance and billing can be confusing and we want to help minimize any problems you may experience regarding payment from your insurance company for your vision. Due to the hundreds of insurance companies that offer vision benefits, we realize the need to inform you of our policies regarding insurance filing.

Effective January 1, 2007 Lifetime Vision & Contacts will accept the following insurances:

• Vision Service Plan (VSP)*
• Davis Vision*
• Spectera*
• Eyemed
• Medicare
• Blue Cross/Blue Shield
• Aetna
• Mamsi/Optimum Choice
• Alliance

*Includes All Federal Workers

If you have the insurance listed above...

Our staff will collect any applicable co-payments and charges for non-covered services from time of service. We will file a claim for services to your insurance company and expect to be reimbursed from the insurance company. Please be aware that payment from your insurance company cannot be collected and benefits are not determined until your claim is processed. You will be financially responsible for any services your insurance company denies.
